Please report this information by first selecting the reporting period and then filling out the following form in its entirety.
The number of patients admitted to the hospital.
Inpatient days of care means a period during which a patient received inpatient services with: the time between the midnight census on two successive calendar days counting as one period, and the day of discharge being counted only when the patient is admitted and discharged on the same day.
Count the number of visits to the emergency department. Emergency outpatient can be admitted to the inpatient areas of the hospital, but they are still counted as emergency visits and subsequently as inpatient admissions.
For designated Trauma 1 hospitals only - count the number of visits to a separately-indentified trauma unit during the reporting period.
Count each patient undergoing surgery as one surgical operation regardless of the number of surgical procedures that were performed while the patient was in the operating or procedure room. The patients included in this category are admitted to the hospital.
Count each operation performed on patients who do not remain in the hospital overnight. Include all operations whether performed in the inpatient operating rooms or in procedure rooms located in an outpatient facility. Include endoscopy procedures only when used as an operative tool and not when used for diagnosis alone.
